its muscular coming from a large muscle we have at back of neck just under the brain it can traverse up leading to headaches when we sing/orgasm. or to shoulder blades. i bet breating in is worse. a shooting pain between shoulder blades at times ? easy sorted with a muscle relaxant from melleril.. its semi stress related. I bet some days you feel you cannot move your neck ? or certain ways you posture your neck eases it ?
muscular and easily sorted.
Failing that of course, spending a little time off ones back now and then might aleviate the symptoms.

Have you checked yourself in a mirror recently for any sign of an object sticking out of your back??? This pain could be caused by somebody having stabbed you with a bloody great big carving knife, or similar.
If you see a handle shaped object sticking out, this might provide a slight clue.
